Need Help ASAP

I been dating this girl for about 4 months. And I told her to get tested and all she did was a rapid hiv test using her finger and came out negative, but I find those test not accurate.

My questions are:

1. I fingered her vagina with 3 of my fingers and when she got the orgasm I used the same fingers to grip my penis and to put the condom on my penis. Am I at risk?

2. I accidentally inserted the condom the wrong direction on my penis, but quickly realized, could there be any semen on exterior of condom to get her pregnant?

3. She rubbed her vagina with her own hands and used her wet hands with vaginal fluid on my penis while giving me a blow job unprotected?

4. We used many condoms, but the one I was unsure about were the 40% ultra thin condom and ribbed condoms are they less effective?

You never had a risk of contracting HIV.

Not accurate? Really?  Why?

You told her to test? What about you?

Questions 1 to 4 are not an HIV transmission concern.
