LVEF 30%,weakness and gradual weight loss

My Father-in-low, aged 74 undergone bypass surgery in Oct 2009 in one of the renowned hospital at Kolkatta. He is a diabetic patient but with medicine was  under control. After two months of bypass surgery, he developed infection (pre-sternal obsess) for which he had to re-admitted in the hospital. The surgery was reopened and high dose antibiotic were  adminstered after medication he was released but by that time his health was detoriorated, but more or less was normal. Since than he has developed many side effects like eye problem, ear problem, hedache etc. Several tests have been done since then but the findings are found ok except for LVEF at 30%, BP is normal (120/80)

His health is detoriorated substantially, he is now almost bed ridden due to weakness, his weight is now 42 kg( at the time of bypass 72 kg) and health condition is worsening. We are clueless as to what is cause of this ( he was a very healthy man otherwise befor bypass, non smoker,no drink..). Consulting so many doctor but not getting a sloution.  

He is taking the following medicines since last one year or so :

a)Nitrocontine 2.6 ( daily at 8 pm)
b)Coversyl 4 mg ( 1 at 7 pm)
c)Urimax 0.4 ( one tab at bed time)
d)Lanoxin (1/2 tab 5 days in a week)
e)Clopitab 75 ( twice daily)
f) Humna Actapid (5 units , thrice daily)
g)Vit A to Z Gold ( One Daily)
h) Aldactone 25 ( twice daily)
i) Tonact 20 ( One at 7 pm) - this is stopped since last week
j) Sturazom Forth  (twice daily) - New medice started last week

Recent Blood Sugar reading is Fasting ~ 110 to 135.

KIndly help/suggest  how to improve the patients condition....
