NGO Graphy

I am 45 male and 19 January 2014 I got heartattack and now after months I had my NGO graphy test, here is the report please tell me what to do.

Left Main Stem: mild irregularities, bifurcating vessel
Left Anterior Descending Artery: tight mid stenosis followed by moderate lesion at mid distal junction. fair sized D1 shows severe ostial disease.
Left Circumflex Artery: moderate to tight proximal stenosis followed by total distal occlusion. small sized OM1 has total proximal occlusion.
Right Cornary Artery: dominant vessel with total mid occlusion. distal vessel fills via bridging collaterals as well as from left system.
LV Angiogram: antero apical hypokinesia with moderate LV systolic function. ejection fraction = 40%

Your left anterior descending has a tight blockage halfway down. Your diagonal 1 vessel (D1) has a blockage. Your left circumflex artery has a tight blockage at its source and a total blockage near the bottom. Your obtuse marginal 1 (om1) vessel has a total blockage. Your right artery has a total blockage halfway down but is being fed into the bottom by the left side. However the left side is also becoming quite a mess and so this feed could soon stop. Your EF is already down to 40% and so any more loss of heart muscle will result in heart failure.

It looks as though you require bypass surgery
