I am 58 yr old male. I get a annual physical every year.  My ECG shows a slight elevation. I am a very active person. I run every day.

Again, this would have to be interpreted by a trained expert because it depends on other factors such as the shape. An elevated ST segment can reflect all kinds of issues such as ischemia. If mild and at rest, such as the case of when an ECG is taken, then I think the Cardiologist may want to run further tests. Now saying that, the fitter you are, the more likely you will see abnormalities in an ECG, including elevated ST-segment. It is classed abnormal simply because they are not the same as the average person, not because there is a fault. Most athletes will display raised ST segments because the left ventricles are larger and have thicker muscle. You run every day, so it may be the reason.

This must be one of the most difficult questions I have seen posted. I say that because there is no information apart from 'slight elevation'. Your ECG shows all the cycle of the heart, from polarization/repolarization of the Atria and Ventricles, the delay between repolarization of Atria and activation of the Ventricles. It shows how long the Atria and Ventricles take to relax among other information. I would have to ask which part of the trace is elevated slightly? what shape is it?
