Is it severe heart burn or heart attack?

Earlier today while grocery shopping I became extremely fatigued. Before which I began to notice pain in my chest that radiated towards both clavicles and upward into my throat. No acid reflux was present. I continued with fatigue, dizziness, general feeling of not being well and the pain continued to radiate - coming in waves and increasing in intensity.

Within an hour the pain was widespread throughout my upper body (chest, esophagus, clavicles, both arms, shoulder blades, upwards into the right side of my neck and jaw).  As the bouts continued the intensity also continued and was accompanied by periodic heaving/vomiting (not due to nausea but due to the sever pain spreading into my throat and triggering my gag reflex).

Later in the bouts - the central location seemed to settle into the bottom of my throat and extreme upper chest. The pain would begin and spread, increasing in intensity as it spread. Within 5-10 minutes it would begin to subside only to return.

Now is the 4th hour from the beginning of the episodes - the coughing and gagging has subsided some, but the pain across both clavicles and shoulder blades continue.   Is this more related to severe heartburn than to heart attack? (at the height of the pain I would rate it at a 8 ) - as of right now I would rate it around a 4.
