From a young age onward it takes some many years to build up sufficient a blockage to course problems. How many exactly? That depends on the the things that where already answered by larrye re: smoking, high Cholesterol levels, Alcohol, inactivity and so on.
It took me to 53 years of age to find i had clocked Heart Arteries of 70 to 80% and for me to have a triple bypass done.

I'm not a doctor, but this is very variable. It really depends not solely upon your smoking, but upon your diet, your lipid levels, and upon your bodies "predisposition" to manufacture plaque. I wouldn't even know where to start answering your question.
