pain when i clench my buttocks

I have had piles and anul fissures( tears in skin recently). And they where almost gone now im experiencing a bit of that still but mainly a really bad like muscle strain when i pass stools and when i clench my buttocks and sometimes even when i get up from a chair even know im not really constipated and they are relitivly soft. What could this be

Doctors can't give you a full response to your question because they can't see whats wrong in person. GO BACK TO YOUR DOCTOR.

I have just had botox in a fissure (tear) up my bottom. And i asked about my piles as i believe they where a real problem aswell because the fissure was getting better on its own and they said it looked like i have piles but not massive or loads of them. But as i told them im convinced they are what are giving be constant pain and itching and disconfort but they said they probably cant opperate on them as it can make them worse but they told me nothing i said i cant live like this and as most surgeons do in my and many other peoples appoion he just sruged his shoulders. So what happens now as i would have to do something stupid if this is the case as i couldnt live with the piles like they are he didnt even say to put creams on although they havent helped much anyway and do i have to live on laxatives. Also i have been told that botox dont work always work for fissures and can make them work be he didnt say this as i think he just wanted to go home and collect his paycheck.Please reply urgently at im at the end of my teather

The pain in the anus can be due to a tear inside, fissures or due to infection. It could also be due to an abscess. Infected piles or hemorrhoid is another possibility. At times such a pain is due to compression of spinal nerves. Infection with herpes simplex virus can also be another cause. IBS can also cause a feeling of pressure in rectum. It is difficult to comment without examination.
Please consult your PCP for primary examination followed by proper referral.
