Hep B / Passive or Active / Alinia

Dear All,
I have been diagnosed with Chronic  Hepatitis-B in 2005 in a blood donation Camp .So I was directed to hematologist . I had taken all the tests including HBV-DNA PCR which was undetectable at that time while HBsAg was positive and HBeAg was negative. Also SGOT & SGPT was normal. Also I was giving blood test for every one year which shows all the results are normal.

In July, 2009 I gave another test which showed that HBV-DNA was in detectable range. I'm giving all the results which I had taken twice once in the year 2009 & another in 2010

July-2009 Report

Hbs-Ag - Positive

Hbe AG - Negative

SGOT - 36.0 ( Normal : 5 to 35 IU/L )

SGPT - 24.0 (Normal : 0 to 40 IU/L)

HBV Quantitative PCR - 9,750 copiles/ml

Feb-2010 Report

Hbe AG - Negative

SGOT - 25.0 ( Normal : 5 to 35 IU/L )

SGPT - 29.0 (Normal : 0 to 40 IU/L)

HBV Quantitative PCR - 3800 copiles/ml ( 537 IU/ml)

My Questions :
1) I want to know whether I'm in Inactive stage or Active stage.
2) Whether Treatment is needed now?My doctor said that he cannot start treatment unless there is  abnormal fluctuations in SGOT & SGPT.
3) Whether I'm affected with Chronic mutant hepatitis B ? Liver Biopsy is needed?
4) Can I try Alinia ?. I'm in India . Can I try Cipla's NITARID 500mg. Really Alinia is working for Chronic Hep B ?

1) Do Nizonide500 have any side effects?... Whether It damages liver further?
no sides but since it is also active against bateria it might lower healthy bacteria in the stomach and make mild transient diarrea so nothing to worry about.
i suggest use of probiotics like when you take antibiotics to balance stomach bacteria especially if you want to try the high dosage 3 pills per day, on trials it has been used without sides until 4000mg for short periods like 15-30days but i strongly suggest the maximum dosage of 3 pills per day

2) how many tablets I shud take daily?
2 or 3 with food.food is important because makes more nitazoxanide absorbtion in the blood flow and keep less in the stomach (breakfast, lunch and dinner)
take the probiotics before start so you avoid diarrea

website is not clear , i guess it is mine is called nizonide500 and made by lupin and ind-swift, it guess one of these two, the difference maybe one has less than 500mg NIZONIDE DT TABLET or NIZONIDE

do not take the ones with OFLOXACIN which is an antibiotic and antibiotics damage liver

this is the one i take and that i checked purity

probiotics are the healthy bacteria in the stomach, you must look for antibiotic resistant probiotics (spores are usually antibiotic resistant)

try to translate and find the same generic prodoct in india, they usually must be taken when you take antibiotics to avoid diarrea

1) I want to know whether I'm in Inactive stage or Active stage.
active carrier with inactive hepatitis for the moment (inactive carrier of hbsag <500iu/ml hbvdna und alt below 30)

2) Whether Treatment is needed now?My doctor said that he cannot start treatment unless there is  abnormal fluctuations in SGOT & SGPT.
there is no official therapy that can help you.since our biggest results on nitazoxanide (alinia) i'd start it right away since your condition has usually low hbsag so it should be easy to make it negative.nitazoxanide is the most potent drug on hbe negative we ve seen until now.
it is off label, your doctor will not be able to prescribe it.nizonide500 by lupin-ind-swift is the most pure generic, made in india so easy to find for you.it s marketed for diarrea.

3) Whether I'm affected with Chronic mutant hepatitis B ? Liver Biopsy is needed?
no because you have no liver damage since all test in previous yaers were normal, mutations and genotypes are not important because nitazoxanide makes no rresistance.only hbe negative is important for nitazoxanide to act fast

4) Can I try Alinia ?. I'm in India . Can I try Cipla's NITARID 500mg.
we have not checked purity of nitarid 500, i have found an article online and looks less pure than nizonide500 although we should check nitarid too and not trust online articles only.if you don't find nizonide 500 go for nitarid 500

Really Alinia is working for Chronic Hep B ?
look at our reports

if you start nizonide500 you mist check hbsag quantity by abbott quantitative hbsag iu/ml and hbvdna pcr before start and after at 4-8-12 weeks
you might have little alt elevation at start it is a good sign nothing to worry

please report blood tests and improvements to the community so that we can share progress and results of nitazoxanide
also start probiotics as i said in other posts to prevent diarrea if you go on 3 pills per day
