4 week visit vl <43

Well at week for vl less than 43 ,I think I got the symbol right . Platelets and RBC dropped soo lowering peg from 180 to 135 mcg . Redo on platelet count Monday and again in two weeks . When they will do another vl test also .otc cream for the rashly hive thing . That about sums it up . Doctor said the lower dose of peg should not have any relevance on outcome of treatment . Has anyone else had to lower their peg ?

Glad to hear your seeing a Hepa. And yes interferon will cause your WBC to become lower. I would ask him about giving you a dose of Neupogen. It's fast acting in raising you ANC number which is the main concern.

Sorry forgot , yes only twelve as of know ,if meds do not change .while talking about options prolonging treatment on lower dose is another avenue. One I do not wish to travel . Keeping my fingers crossed  next labs are ok .

Yes as I just posted above Hepatologist at R.I. Hosp. Clinic. I believe they are more concerned with red and white count a little more than platelets. We knew they were going to drop going in to this tx . Along with skin reaction think meds might be a little much for my body . Upset ? Yes ! They did say the lower dose should not affect outcome . However they did not say that about RIBA which they will lower if my count had another big drop . I take it your doctors are more aggressive ? Should I push for more time to see if I level off ? I know they are not big fans of procrit . They say blood transfusion and adjust meds ,because the procrit   it takes weeks to work .

Hello , yes I am seeing hepatologist ,actually head of hepatology clinic Rhode Island Hospital , he is overseeing my case .I actually spaced out during visit and forgot to get my numbers. Last platelets were 44,000. But my red and white count had a (big ) drop .along with hives rash reaction he lowered my shot to 135 mcg .redo on VL and CBC with plat. And dif. Monday . Everyone hoping I level off . I will ask about promacta and see what they say .

Just got 8 wk bldwk and am still <15 on VL. Sodium is a tad low. Bilirubin @ 1.9 with an upper ref range of 1.2. Hemoglobin & Hemotocrit both slghtly low but not by much. Any concerns I should have?

Like Pooh, I am also curious on whether your seeing a Hepatologist as they more likely have seen platelets drop rather low before getting to concerned. When I last treated mine got as low as the mid to upper 20's without a dose reduction. Also keep in mind platelets will bounce around somewhat on interferon treatment. Mine ended going back up into the 50's on their own Being I was also cirrhotic the odds are against us so we need every bit of help we can get... BTW I am curious if your only treating for 12 weeks? As pooh said us cirrhotics really need a Hepatologist treating us.

You did not say how low your platelets went down to. I know you started around 50,000 or so.

I am wondering what type of doctor you have. Do you have a Hepatologist or do you just have a Gastroenterologist? Did the doctor discuss giving you Promacta to boost your platelet count (instead of reducing your Interferon dose)?

I would also ask the doctor why he thinks lowering the Interferon dose will not have any effect on treatment success. It is the Interferon that boosts the immune system and it is the immune system that kills the virus. So we need our immune systems working to their highest capacity to kill the virus.

You are genotype 4 and have Cirrhosis. I would be very concerned about lowering any of the medications if I was Genotype 4 and had Cirrhosis. If you do not have a Hepatologist, I would suggest that you get one ASAP. A Hepatologist should be managing the treatment of anyone with Cirrhosis because a  Hepatologist can better manage your treatment and will have a better idea of how to treat side effects and adverse reactions.
