May I ask why you are concerned about a UA?  Noticed that you quit drinks a few years back.  If it's Pot, they may deny you.  Over Christmas I had a few hard candies from Colorado with my 90 year old dad and 87 year old mom.  Not my normal routine but as silly as it sounds kind of a bonding experience with my mom and dad as alcohol is prohibited.
Even tho I went thru treatment and was cured, in order to stay in my Hepatologists good graces it's required that I see a Physchiatrist for my only indiscretion.
If I were in your shoes stay clear of any implication which could endanger treatment even if it seems innocent at the time.  Your life is at risk if you were to be denied.  Be smart and do the right thing.

Apparently, drug addiction doesn't seem to be a problem with the compliance to Hepatitis C treatment.   There are lots of people who have been injecting illegal drugs who have Hepatitis C.  They are a great contributor to the transmission of Hepatitis C.   If we don't treat and educate the drug user….then how are we going to stop Hepatitis C.

Even tho there is much talk about no more pre existing conditions
can be denied….there is a clause found in the "obamacarefacts.com".
its the "One exception: Grandfathered individual health insurance plans."

There is some plans where drug abuse and alcoholism could not be covered. The presents of this would give the insurance company a way to save a lot of money.  If you have a grandfathered plan you might want to
take a closer look and make changes to get the treatment you deserve.

If UA is urine test, I would say yes. The last thing my insurance did before issuing Harvoni coverage was ask for urine test. The very 1st part of treatment for liver disease should be to be totally clean and live a healthy life style. No medicine will work without 1st doing that.
Harvoni is very expensive and the insurance companies do not like to spend money.

Typically not, although it may be a criteria through your insurance company.
