I have to agree with John, that sometimes In-Patient Treatment can become neccessary, since your depression has gone on for two years now,has gotten worse, it includes sleep deprivation, suicidal ideation, and your Effexor is no
longer working at all, and may actually be the cause of your sleep deprivation. Only a Psychiatrist would be able to advise you.
Once your depression and sleep deprivation has stabilized (Psych med adjustment tends to take about 6 weeks, at the least, since often times, one wont work, and another has to be tried, and then a "Med ****-tail" has to be figured out, the whole thing can take as long as 6 months), then you can address the physical symptoms. The problem with Clinical Depression is, it sometimes is the cause of many painful physical symptoms, strange as that may seem. Also, Clinaical Depression is a real challenge for even the best Psychiatrist to treat.
I know my sister takes Effexor, and two other meds as well, to treat her depression, and she is a R.N. When she was your age, her Effexor was no longer working either. She did have to go to a very good 30 day In-Patient Program, in our area, where Doctors adjusted her Meds, and she participated in lots of group therapy. I also went to a In-Patient Program, for 30 days, 13 years ago, to treat my own depression. I got some very good counseling at my Program, as well.
I noticed from an earlier post you have utilized the VA which you stated was a long drive for you.
treatment can make other problems persist. In my case I had a history of deppression before I first treated in 2007 and although was on cimbalta during treatment, Post treatment it got worse and I suffered for a few more years until It got to the point I went into in-patient in a behavior health center. I was put on a stronger med and things like pain and tiredness along with my sleep patterns got much better,
Some things possibly in your case may be purely physical like the pain.
You said the pain was in your Right leg, Have you had an MRI as that could be indictave of a possible herneated disc, not saying that is the case but you seem to have many issues and each should be evaluated by a specialist. One great doctor really cant handle physical and mental issues but I would start with a behavior health professional to have the depression, night terrors and sleep issues evaluated and go fron there.
