Is this herpes?

Hi Everyone

Thanks for reading. Like many, I'm pretty stressed by the unknown.

I'm trying to figure out if my symptoms could be herpes. 1 week ago I had unprotected receptive oral sex with manual release. The next day, within 10 hours, I developed aching in the penis shaft. This aching has persisted and worsened every day with periods of intense burning (?urethritis). I have no visible lesions of any kind. I have no discharge and urinating is entirely painless. The pain occurs after urinating (minutes or more) and intermittently unrelated to urinating. It can be very intense at times and eases off to almost nothing at others. I saw a urologist (a friend) who did a prostate exam, PSA, urinalysis and culture. All normal/negative. I asked about STI testing, he will order the tests but feels this is more likely chronic prostatitis/complex pelvic pain. I've had a Dx of chronic prostatitis for decades but it's been quiet for many years and the proximity of such intense symptoms to a sexual exposure makes me question the cause.

I know atypical cases of anything are possible. I'm wondering here if the absence of lesions, the fast onset of symptoms, the absence of any discharge, absence of white cells on urinalysis, and the complete absence of pain on urinating could still be compatible with a genital HSV1 infection. Without external lesions, I'd expect internal lesions (herpes urethritis) to cause significant pain on urination, as I've read others experienced). I did take 2gm of azithromycin early on without any improvement.

Congrats on the neg testing. You have zero chance you contracted either syphilis or hiv. You're all good

Update - after 2 weeks the penile symptoms just resolved, completely. So now have prostate non tender with normal, PSA normal, urinalysis and negative urine culture. I have also had testing for Hepatitis A, B, C, Chlymidia, Gonnorrhea - all negative. Syphilis and HIV early testing pending.

I agree. plus that is not a big issue from oral sex.

It does feel similar to past prostatis symptoms but that involved more deep pelvic pain and much much less intense penile discomfort. I've heard chlamydia can have no discharge but again, it was quick in onset and eberything seems to need a day or two of incubation time.  As well, I did take antibiotics that cover chlamydia. Thanks to you both for your comments. I appreciate it.  If it wasn't for the proximity in time and the severity, I'd say prostatitis for sure. There's huge psychology overlaying these problems I know, even when trying to be rational.

To answer your question no. If you did have lesions in the urethra you would most assuredly have some discharge and very painful urination. Plus it would be very rare to have them inside and not on the outside as well. Your symptoms came on way to fast to be an STI. Not to mention hsv from a single exposure to oral sex when no cold sores are present would be a long shot.
Add everything up and hsv does not fit at all. The only two other STI's that are a possibility from unprotected oral are NGU and gonnohrea. I probably spelled that wrong. I always do.
Without discharge and burning urination I would not think this is the answer either. Again your symptoms came on to quick. Does this feel like the other problems you've experienced in the past?

Hi, no std shows symptoms the next day so its not related to the oral sex.
