An infection will definitely raise your creatinine, especially if you're taking oral meds and even more when Drs give you IV antibiotics. Yes antibiotics can be very toxivc to our system, yet they work to kill whatever bug is hurting us.
How old was your donor? Was your donor alot older than you? is that what your Dr means by "your kidney is old" ?

my tacrolimus level is :22.5 and my Weight is 75 kg ( 31 year)
i Consume 3 tacrolimus ( 1 mg) in 24 hr
i do not drink caffeine.

What do your Drs say about that? how long has this been going on? Are you taking any antibiotics, diuretics or any new drugs regimen? Are you drinking plenty of water?
You know that tacrolimus is a great and powerful drug which prevents organ rejection. BUT, if the tacro concentration in the blood is too high, then it can damage the kidneys.
You need to make sure you stay well hydrated, drink lots and lots of fluid, try to avoid caffeine or limit it, because caffeine causes dehydration.

Avoid as much sodium as possible, keep blood pressure close to normal levels (if that requires medicine, then take it as prescribed), drink plenty of fluids ( at least 64oz/day) do not take any OCT NSAIDs (over the counter pain relievers, which contain ibuprofen, or aspirin)
