What is causing my penis issues for over a year? Losing my mind.

Hey,
I've been dealing with issues on my penis for quite a while now. It has affected both the foreskin and the head. On and off problems for maybe 2 years already although in the past year it's been worse and more frequent. It's really been affecting me mentally and my sex life. A year ago I got a swab test and was told it was enterococcus bacteria. I was prescribed clotrimazole and it went away, but then it would come back. I came to the conclusion that it must be balanitis or a yeast infection being passed between my girlfriend and I. I thought it was a yeast infection because it would flare up after eating certain foods(I have an history of internal Candida Overgrowth. I tried alot of different home remedies. Monistat, coconut oil, antifungal/antimicrobial soap, etc. I bought new 100% cotton knit boxers the other day and am going to see if that makes a difference.

I went to the primary doctor again and they gave me Clotrimazole again. The day I went it didn't look as bad as other day and she could barely notice it. It seems to be working somewhat so far, although my penis still isn't looking normal like it used to before I started dealing with all this. There seems to be alot of dryness on the head, and when I pull my foreskin down it seems "glossy" or "shiny". Don't know another way to describe it.

Here I am providing some pictures of the actual redness, itchiness, bumps, and minor inflammation I dealt with before I started using the cream again and then pictures from today of the dryness I am noticing again. I am not using any body wash or soap that may be affecting of any sort.

I finished the clotrimazole cream after 14 days a couple days ago. The red irritation is gone, but the discoloring/dryness/visual that my penis is going to peel is still there. I masturbated for the first time in months and then this occured. It was also already looking weird before I masterbated.

I am please asking for any guidance or help on what to do, how to treat this, or who to see about this.

Mine looks almost identical to yours. Did you ever figure out how to heal it?
