Feeling of needing to poop always & lower back pain

I'm 28 yrs old male. I have varicocele for more than 7 years, but so far no big deal except some pain on strenous activity and sitting for long time. However, recently I had "terminal dribbling" issue and I visited Urologist and did MRI. Nothing was found in it except some prostate calcification. No prostate enlargement, no issues in liver or kidney. However, I feel lower back pain, have terminal dribbling issue, always fatigued, and sometimes pain inside (probably inside perineum). I thought it to be a prostate issue, but MRI shows nothing except some calcification. I once also had sharp pain inside when I sneezed. Is this all about any issues like prostatis or anything else? Urologist is wondering that the results show nothing wrong. Though the urine culture results before few months showed minor infection and it has cleared now in the recent test. But, still I have all the issues mentioned above. The worst part is I have a feel of something inside, I don't know whether its poop or anything to do with prostate. This has been reducing my quality of life very badly. Moreover, I was down with fever which cause low T (now back to 550+ ng), and caused heavy muscle loss. I don't know whether this is related to any prostatis flare up. I don't have any history of UTI. Doctor says everything is normal. I don't know whether it is hemorrhoids. Now and then I have burning anus when I poop. But I don't know why I have these issues. My nature of work makes me site for more than 9 hours.

I meant IBS = Irritable Bowel Syndrome.

It could be something as simple as IBS (Irritable Bowel Disease). You can try some OTC
meds like IBGuard or Imodium and see if that helps at all. Good luck!
