Ok so I am sitting here with both of my big toes tapping away when I found your post. too funny.
It sounds like a muscle spasm to me. I know when I am having random spasms in my legs, stopping the spasm with thought causes me to notice there is a tight muscle somewhere that is causing the spasm.
I know Baclofen helps me a lot with this. I often wonder if I am going it on my own or if it is just happening. I guess voluntary or in voluntary I call it a nervous tic lol. I can stop it, but it does take thought like you said.
Tell you Dr again, this can be relieved. Stretching out the tight leg muscle may help too.

They both move on their own. Just ut and down, as if tapping out a rhythm.
I can make them stop with effort, but I really have to concentrate.
I can feel the muscle up the front of my leg when I make them stop.
If relaxed, or not thinking about it, they do this whether I sit or stand
I have been worried about it, I can't help wondering why they do this?
I told my doc about it, he didn't respond.
So please tell me what you know about this!

My legs and feet dance to their own drummer. I wear basic athletic shoes with nothing special about them so my chance at falling is decreased.
Hopefully your big toes are not a sign of things to come. Are you taking muscle relaxants to control your spasms?
Don't give in to the pressure. My sister (who I love) keeps showing me fashionable sandals in different styles that she thinks I could wear. A couple of them are clogs.
Just say no:-)
