Can you have muscle twitches and spasms along with parasthesias on left side, some on right with extreme fatigue, still have muscle strength no obvious balance issues and have ms.

The short answer is: Yes, you can.  It might be considered an atypical presentation and could be related to other diseases like ADEM or Lyme's.

Muscle twitches, spasms, parathesias, fatigue - no muscle weakness and no balance issues are all rather non-specific symptoms or lack of symptoms. Meaning, those symptoms can be attributed to many different diagnosis'. Most of us here (I am undx'd) have been through many assorted tests to rule out the MS mimics. If you look at the "health pages" on the top right side of the screen, there is a lot of really good information there. :)

That said, yes, you can still have MS even without losing muscle strength or balance issues. The location of the lesion(s) determines the area of symptoms but, as many will say here, it's nearly impossible to map out the lesions to the symptoms.
