Hello doc, If I were to have peripheral neuropathy, even though I never had an EMG/NCS, what would be my symptoms.  As I told you before, I have razor burn feelings that pop up at different places all over my body.  I also have twitches, buzzing feelings, but no numbness.  The feelings always make there appearance everyday, they last for a few seconds then gone only to later come back at a different location.  They are mainly under my armpits, around ankle area, knees, and sometimes on the top part of my hand.  My neuro says all my bloodwork is normal, let me tell you, I had a pile of it done.  She said maybe small fiber neuropathy, or post viral syndrome, or fibromyalgia.

Can you comment on this?  What do you think?  I am not diabetic, nor do I have a thyroid problem.  

Could stress cause these sensations?    Thanks, Jen

Hi.

I think you are thinking about epilepsy due to the twitching. Fr this to be a part of epilepsy, you would have to have a brain dysfunction for a few seconds of minutes. Your symptoms do not resemble epilepsy.

A suspected neuropathy should be ruled out or diagnosed by EMG and nerve conduction study. Can you please paste the link of your original post ?

Thank you for your reply.  She did mention a nerve biopsy, but said that is usually the last resort.  I never had a EMG done either.  I think that she is thinking about a post viral syndrome because while I was really sick with nausea and a mild fever for 3 months, the only thing that showed up in my blood work was slightly elevated white blood cells, slightly elevated platelets, and the EBV was a reactivated infection.  It was after the nausea went away that the strange feelings started, and I have been having them for 7 years now.  I have read that small fiber neuropathy is usually idiopathic.  All I know is that it is getting worse every year.  I do have muscle twitching also.

What do you think would be a good step tp make next?

Hello Jenny.

The symptoms of peripheral neuropathy would be paresthesias (burning, tingling, numbness) and depending upon the nerves affected, muscle weakness and or twitching. These symptoms are variable, and can occur in any combination.

Your symptoms do suggest small fiber neuropathy. But your history has an EBV infection mentioned. Probably that is what is making your neurologist think of a post viral syndrome. But I do not see a viral cause here.

You have been through a lot of testing already. Has any of your treating doctors ever mentioned nerve biopsy ? Was it considered ?
