Indentation on the top of my head, and I can't think well?

There is a newly formed indentation on the top of my head. I can't think much at all, I especially have difficulty with mental math and expressing my thoughts verbally.

When I tilt my head back and touch the top back portion of my skull where the dent is, it makes me very dizzy and nauseated. And when the back of my neck/head is exposed to cold air, it makes my shoulders and neck shake violently for around 2 seconds.

What could possibly be the cause of this? Should I visit a doctor? - I don't think I have enough money to.

I also have a hard time seeing, hearing, and feeling things, but I don't know if this is related. And extremely poor spatial reasoning.
