concussion?

i am 19 years old and august 2009 i passed out in the shower and hit my head on the floor. ever since then i am having big pain on my temples on my forehead, and constant headaches daily. and recently on my left temple, the side i fell on, is hot to the touch, and really hurts. i have tried so many different medicines any adivce?

Hi, Thank you for your question. Usually, such headaches and dizziness may result due to post-concussion syndrome and may last for weeks or months after the injury that caused from the injury of a fall, vehicle accident, or sports injury. In addition, patient may experience either migraine type or tension type headache. For treatment approach, first it is essential to get (MRI) done to detect the brain abnormalities after that injury. Treatment commonly may be suggested for headaches such as pain killers, some antidepressants, physical therapy and regular exercises. If you suffer continuous headache, please arrange an appointment with a neurologist right away who will evaluate the possibilities here and can provide you an appropriate treatment. Further referral to a psychologist may be also helpful in order to treat usual anxiety or depression after such injuries (sometimes one may have) that may be one of the aggravating factors for persistent headache. One of the big questions asked in an emergency room is that did you lose consciousness first and then fall, or did you simply become weak and then hit your head?

After all this time the condition had stabilized. That means that you don't have to run to the emergency room.

There is a possibility your "passing out" was due to something simple, such as simple systemic vasodilation, or something complex and ominous.

It is unlikely, but possible you have experienced a minor stroke. Either from a clot or a bleed.

Your complaints warrant an MRI and an MRA. An MRA is used to look at blood vessels and passageways. I am a little bit concerned that you may have had a minor stroke last year and may have impending symptoms of another one. When you are nineteen these events are unlikely, but I am painting a "worse-case" scenario.

There is nothing really critical about this, but sometime in the next few days would do fine.

Of course a consult with a neurologist is in order.
