coordination / jerky movements in right hand & wrist (youtube video included)

Same problem in my right foot and ankle but it's not as significant. I'm not sure when it started, but within the last 2 years. My main hobby is piano and guitar, so the lack of coordination and overall slowness in my right hand is pretty demotivating. It's worse with guitar, which is why the first movement in the video is mimicking a vibrato motion with wrist. Anyone know what this is and what I can do about it? Any help appreciated.
