How do you get ball of the foot pain all of a sudde

I’ve had bunion and hammer toe surgery. All good until about 4 months ago. Now the pain in the ball of my foot is so bad I cannot walk barefoot. Had orthotics made still bad. How does it go from no pain to excessive pain All of a sudden ?
