20cm cyst on right ovary

Hi I was told I had PCOS 12 yrs ago and since then hav had approx 2 periods and lots of aches and pains, last week I had a clear discharge with blood streaks in so was a little alarmed and went to see my doctor, she examined me and said my womb was enlarged and referred me for an ultra sound scan, I had that done today and the sonographer told me that I have a 20 cm cyst on my right ovary, she didn't tell me what kind of cyst she just said that I need to go back to my doctor to get referred to see gynocologist, I got in to see my doctor 4 hours ago and she is referring me to hospital to have the cyst surgically removed what I need to know is that can I have operation while awake? I'm scared of being put to sleep.... Also how long will I have to stay in hospital and how long does it take to recover from such procedure? Id appreciate any advice that I can get, thank u for reading x

Hi Julianne,

Try not to worry, I know its hard, but I worried myself over nothing and read alsorts on the internet, my cyst looked very suspicious and turned out completely fine. I am sure if your doctor wants to wait for 6 weeks, the cyst must not look suspicious on ultrasound, I know no-one can say for sure until they are removed and tested, but they get a pretty good idea.  Most cysts 95% are benign, even complex ones

I'm so glad to find this forum.  I had an ultrasound on Monday, then a phone call from gyn on Tuesday to tell me I had more than one cyst on the right ovary.  I had breast cancer almost 11 years ago so I am very worried.  My doctor recommended that I wait 6 weeks and repeat the ultrasound.  If cysts are still there, then surgery.

Please say a prayer for me.  Thank you!

Hi, my experiences was a little different,  My GP referred me to the hospital (not for a scan) but for fertility testing, so I was sent to a Gyno, he then gave me an internal examination, where he felt an enlarged ovary (but he didnt tell me at the time, maybe not to worry me) and just said afterwards he was sending me for a scan to check my tubes werent blocked and to check my ovaries,  I thought nothing of it and went on my merry way.  I got an appointment for a scan for a month later.  It was only from the scan that it then was turned around extremly fast.  The Sonographer referred it as urgent, ( I didnt go back to my GP, I just got a letter direct from the hospital the following day (after the scan) to say I had an appointment with the Gyno the next day! I then panicked a little.  When I met with my Gyno (which happened to not be the same one that gave me the orginal internal) he sat me down and asked me the following:-

Did I have heavy periods, was they painful, was intercourse painful, then based on those answers and the report from the ultrasound ( which suggested an endometrioma) he said I was likely to have endometriosis and the cyst was an endometrioma which was a collection of old blood, then said he needed to book in with his secretarys my operation to remove it, at the time he said 4 weeks, but I got the letter and it was just under 2 weeks.

I would envisage that your appointment will be to first discuss the cyst with your Gyno, he will explain the best way to manage it - ie the type of operation, when it will likely to be etc, this will also give you chance to ask all the questions you want answers to .  I would write a list of questions that you have.

Hi thanx again for replying, it just sounds so scary to be rushed thru everything so quick, I had my scan on Tuesday just gone (exactly 1 week after first seeing the doctor) I also saw my doctor Tuesday night after scan, she phoned the hospital then phone me with vague details of appointment, but I wasn't informed if this is appointment for op or to have a talk with gyno? X

Also just to add, I am not sure they will allow you to stay awake for surgery, I have read some people having spinal epidurals instead of being put to sleep, and their recovery has been alot quicker, I guess this is what they do when the give a c section at delivery of a baby anyway! But depends on the Gyno/surgeon and if he and the anesthetist will agree to it, maybe if you explain your past experiences they might.? Best Wishes. x

I was put on rapid access - from my scan to my first operation was 2 weeks, my scan report was marked as URGENT in block capitals!.  On the NHS they have a 2 wk window to turn something they class as urgent around, and it can be because of the following:-

Size of your cyst - this could cause your ovary to twist on itself causing pain
Cyst could burst - causing pelvic infection
Cancer - please dont let this scare you.  I was turned around very quickly as my cyst was classed as large, and everything has turned out okay for me.

Only think I would say is, I saw my Gyno within 2 days of my scan, and then was having the operation 2 weeks later (abliet he only ended up draining it!).  If you are having an appointment next week with your Gyno, I would then say from there it would be another 2 weeks before they operate, so it will be possibly 3 weeks in total.

Had call from my doctor and they have got me an appointment to see gyno next week under the ' Rapid Access Investigation Service '
Can anyone tell me what this is and why it's been done? Thanx

Orh I feel for you, I was just the same as you and found this forum helpful, but dont make the mistake I did and start reading and doing your own research yourself on the internet, some of it is good, but I ended up self diagnosing myself and reading alsort of horror stories thinking that what was going to happen to me.

I am sorry to hear your past experiences have not been good, My last one I did wake up still with the tubes down my throat, but they soon took them out, I seem to wake up sooner.  I think some people take a while to come round, as it affect people in different ways and makes them sleepy, I come round straight away but then go back to sleep afterwards.

You will be in safe hands, and its easy for me to say all this, but that cyst does need to come out rather than stay in and no surgery is nice unfortunately.  

I think you should insist your doctors sits and talks with you, tell them your concerns and ask them all the questions you want to know answers to.

Hello and thank u for responding, I haven't been told if cyst is liquid or solid so am a little unaware at this point.
Te main reason I don't want to be put under is because I have had that done a couple of times in past for other reasons and always had trouble  waking ( took me longer )
My doc is pushing everything thru so quickly that it's scaring me TBH.
I just wish I knew more as don't like to go into anything without knowing as much information as I can, and I don't know where else to get advice from?
As u can imagine I was pleased to find this forum because I feel like I'm not battling this alone xx

Hi Katy,

Sorry to hear you are going through this, I know its stressful especially if you never have had surgery before,  I was in the same boat as you and come through the other side completely wondering why I fussed over being put to sleep.

Depending on the size of the cyst, some of them can be removed via Keyhole surgery (Laporoscopy) which usually is a day visit to the hospital and I found relatively pain free and came home the same day with just a bit of wind pain.  I had a 10cm dermoid cyst, considered large, but not as large as yours, so I sympathise with what you must be feeling, are you having pain already?

They only orginally drained my cyst as it wasnt supsected to be a solid dermoid, but 6 weeks later it was showing 7cm solid on scan, so I was scheduled to have open surgery (Laparotomy) to remove it 11th January, until I then found out I was pregnant!, it was postponed until my second trimester and I had it removed last week.  

I orginally had all the same fears as you, being put to sleep etc, however I can say the first time I was put to sleep was like a light switch going off and you wake up a second later, with no sense that time has passed.  It doesnt hurt, and it feels over very quickly.   I was asleep for 2 hours with my keyhole as I also had dye put through my tubes to check for blockages.  Just recently although it was a bigger operation I was only under for an hour, as he just went in and took my right ovary, I told him not to bother trying to mess about saving any good tissue as I wanted to be under less time that I had to for the babys sake.

I didnt take any pain meds, neither did I take morphine, again my choice, so I did feel some pain from this operation more than what others would do, but the pain was not so much from the incision but again from wind created from the operation working through my intentines.  My muscles felt like I had done lots of situps, I was up walking the following day, my advice it to walk little but often.  I found it painful to cough, laugh or sneeze, but holiding a pillow to your stomach helps, although I felt pain the recovery has been fast (I am just over a week from surgery).  It gets better everyday and I can promise you that, as long a you walk little and often, you are not advised to lift anything heavier than a kettle for 6 weeks after.  I was in hospital for 3 days, and I have the next 4 weeks off work, and also not permitted to drive.

My incision is 5 inches long across my bikini line and is much better than I expected, my Gyno must have a really steady hand, its finer than a cotton thread and healing really well, some parts of it you cannot even see where the join is, much better then some of my friends c -section cuts when they have had a baby!

I suspect due to the size of your cyst you will need open surgery, but I dont know what type of cyst it is, it may be possible to have keyhole if its a fluid filled cyst, they can drain it then remove the sac thorugh the keyhole, but I would say this is not likely given the size.  

Please dont be worried about being put to sleep, I was the same, and I am a big worrier and looking back I found that the least of my worries, you are there one minute, gone the next second, and back again so quickly and they are telling you its all over.
