Neoplasm just means "new growth." 9cm is large but it can shrink if you give it a couple of months. Mine did.
I had a hysterectomy 6 years ago and saw a physician for pain on my left side, my ultrasound says 9.0 x 4.6 x 7 cm complex cystic mass left hemipelvis, could represent cystic neoplasm from ovarian remnant. I am freaking out- is this too big to do laproscopically, and does neoplasm always mean cancer?

If they have to be removed it is either via laparoscopy (small incisions) or via laparotomy (open abdominal surgery). That could mean just removing the cysts or removing one or both ovaries with them.
The first procedure is usually outpatient or overnight hospital stay with about 2 weeks of recovery, or more. The latter is around 2 to 3 nights in a hospital with 6 weeks of recovery.
Much depends on the size of the cysts, your age and cancer risks in weighing weather to take the "wait and see" approach, or a more aggresive "remove it now" approach. Even simple cysts could need removal if they are large (10 cm or greater is the general rule). The complex mass is something that needs to be better defined. There's always the possibility they will go away on their own (not sure about complex) or rupture, which hurts big time.
Smoking and any other lifestyle changes don't seem to make any impact. At least there's no info out there that tells us how to increase or decrease our risks for these problems that I've seen other than birth control pills which shut down ovarian activity. The only thing I can recommend is that if you are old enough to be of menopause age, get rid of the ovaries and put an end to problems with them for good. I was post-menopause when my ovaries were removed and I felt no difference without them than I did with them. Never had HRT and never will. I don't trust it.
What has your Dr. advised regarding this report. Does he intend a follow up ultrasound? Many times these are just monitored for a few months to see if they disappear. You need to ask your Dr. what his suggestions are.. If , after 3 months, these have not disappeared, you will probably need to have them removed along with the mass.. But, please insist that your Dr. advise you..
I don't know that smoking would have any effect on them..
