Which is causing spotting: Cysts, Fibroid, IUD or hormones?

For about six months now I have been having off-cycle cramping and spotting. I get a pink spotting for 3-5 days around ovulation then it turns kind of brownish and last through until I start my period. It's really annoying. My cramping usually starts around ovulation, stops for a week then returns in full-force the week before my period. I don't have any cramping during the biggest part of my period, only they return about 5-7 days in. This is annoying too.  

The last few months I feel like my body is starting my period grudgingly. I don't get a flow, just a brown sludge for the first 3-5 days. Then, it's like the seal gets broken and I'm gushing like a stuck pig for 5 more days.

I've also noticed that some times my mid-cycle cramps end with more reddish-brown spotting. Like cause and effect.

Went to my GP a couple of weeks ago and after a pap and a pelvic, felt nothing out of the ordinary. I did request an ultrasound which she approved and had it yesterday. Before I left the exam room, the tech said, "The only things I found were a fibroid tumor deep inside your myometrium and a cyst on your right ovary." When I asked how large she said went to look at her images but I told her I'd just ask when my doctor called.  

Well my doctor called today and said my ultrasound was completely normal. Never mentioned a fibroid, never mentioned a cyst. I asked about the fibroid and she said, oh, yes, most women your age have those. Nothing to worry about.

The I asked about the cyst and she said she didn't see anything about a cyst on the report, that it was probably just a follicular cyst. Why doesn't she know? LOL

I have requested a copy of my images and report and I'm picking them up on Thursday.

Which of these issues do you think might be causing all of this ongoing spotting? Apparently my women parts look healthy but I feel it's unusual to have this begin suddenly and continue on as long as it has. I've had this IUD for 11 years and have have no mid-cycle spotting until now, so why would she pin it on that as being the culprit and not th fibroid or mystery cyst?

Any suggestions on what to do next?

The fibroid isn't what bothers me, it's the fact that the technician told me she'd found a cyst on my right ovary but my doctor didn't seem to be aware of it. The tech spent 1/2 the exam doing doppler and looking at that right side - and I know her so don't believe she's seeing things that aren't there. I'm getting the images and the report tomorrow so I'll know more ten. Thank you!

Well, regardless what the tests show, you are uncomfortable with what is happening.  Fibroids can cause a lot of problems with bleeding and pain.  Fibroids are why some women have to have hysterectomies.  If you don't get the answers you are wanting find someone who will as a second opinion.  No good doctor will take any offense to this.  Go to someone else not in the same practice and take images with you.  Let them form their own opinions without seeing the written findings.  Goodluck.
