Help with chronic pain

i am in pain all day, buy after i have been up for 3 to 4 hours im in severe pain in neck, headache, burning sensation in upper back going into right underarm. anyone else have this or know what this is and how to treat it i have been this way since August 2009 on a work injury

In  May 2009 i had arthroscopy shoulder surgery on right shoulder for torn labrum with capsular shrinkage, the Doctor has perscribed for me Darvon N 100 mg 1 tab every 4 to 6 hours, Skelatin 800 mg 1 tab every 6 hours, and Ambien 10 mg for sleep; besides that i did physical therapy for 8 weeks after surgery, i still do the exercises every day when i first get up, i use ice for 20 minutes and heat for 30 minutes on neck, back, and right shoulder. This pain when it comes on last all day, and usually i get 3 to 6 hours of sleep a night even with all the pills i take. Thank You
