how do I get my pharmacy history?

Hello Everyone,

I have my first appt. with a pain management doc on Monday,(today is Friday). I'm going through the paperwork and it says they want me to obtain my pharmacy history and bring it to the appt. What is this and how do I get it? Anyone? Help!

if you have one of the larger insurance carriers that allow you access to benefits explanations,etc on their site you can set up a password system and access it and print yourself as it will show all scripts filled.I would go with the pharm printed one if possible and it will only take them a few minutes to access and print.Just call before you go,tell them you need a precription history for the last year and when can you pick it up.They have people do this all the time especially for tax purposes.

The one on my ins carrier site shows all scripts filled regardless of if they are a 4 script that is not ran thru ins-or different pharms-but I try to stay with one.So if you had to you could print off both.

Another way to do it, (if you use more than one pharmacy) is to call your pharmacy benefit of your medical insurance. I have to do this once a month in order to get reimbursement from a coinsurance that I have and they fax me a printout.

Hi Lolo,

First, let me welcome you to the Pain Management Forum and let you know that we are happy that you found this Forum.  I need to be sure that you understand that there are NO Doctors on this Forum.  We are ALL  CP - Chronic Pain - Patients that share our expertise and experiences that we draw from our OWN CP Issues.


All you have to do is to call your Pharmacy and ask them for a copy of your Medication History.  They should be able to print it out and have it ready for you OR even fax it to your Doctor for you.  If your Pharmacy is a National one such as Walgreen's, CVS, etc. you can go on-line and get it yourself and print it out. You just have to set it up.  Some of them require the Pharmacy to do something for you to b able to go on-line so you might need to get in contact with them.
