One year old baby not gaining weight

My 14 months old girl was born 2.5 kgs and she has gained weight very poorly since then. She is now 7.9 kgs and 29.5" tall. She looks very thin and weak for her age. She had been going good on her solid intake till she was around 10 months. After that she stopped chewing her food properly and would throw out whatever we would give her. Her food would need to be run in a grinder to make a very smooth paste to let her have it. Now she takes around three meals a day, including porridge, veggies, soups, fruits, dairy products etc. Her milk intake must be around 450-500 ml in a day. But she is not gaining any good weight.

Also, she has always hated tummy time since her birth and skipped rolling over and crawling. She was able to sit at right age and now is able to walk holding on to something. She is not able to raise her up from a lying down or sitting position. Her doctor has recently diagonized her for sever hypotonia and has suggested physiotherapy to help her build some muscle tone.

Please suggest some appropriate diet schedule for my baby girl (we are vegetarian Indians) and more about curing hypotonia.
