Midwives work under OB's, they cant deliver in hospitals under their own license. They are generally more natural. They are nurses with an extra 2 years of training (Certified Nurse Midwives). Whether you choose a Midwife or OB depends on your preferences for labor. Should anything go wrong an OB will be on hand for surgery etc. Since midwives aren't licensed for that.
You have either a midwife or an ob not both and in most cases try to get into a midwife asap its usually hard depending on where you live. I had an ob with my first and midwives for my second and now my third.
Most hospital do midwives now its cheaper for the hospital. But most people have both. Or u have an ob and a nurse who does all your blood work ultrasound and ect but doesnt deliver the baby at least I think.

A midwife is usually who you have to do home births. Midwifes are more natural type practices. Ob is a standard doctor for pregnancy.

When? Any time during your pregnancy I would imagine.
When do you get a midwife?
Two different things. Different levels of schooling and different abilities. A midwife can't do a c section.
