hospital bag for baby and mommy

What do I need to pack for myself? And how many outfits?
What do I need to pack for baby? So far for her I have four very warm onsies (just in case, I'm an over packer) & a take home outfit with socks, jacket and a headband.
Also another question, does the hospital provide mittens or should I bring my own?

I went to wal mart and got travel size stuff for my bf and I such as
-shampoo&conditioner
-tooth brush
-tooth paste
-deoterient
-slippers
-sweat pants(comfy clothes)
-nursing pads!
-nursing bra (if breast feeding)
-if not sports bra
Just pack for two nights if u need more I'm sure someone will run home for u

Bring your own mittens.
I took a few onsies and his blanket. The hospital will provide diapers and wipes.
For you I would take loose comfy clothes, house shoes, underwear cause hospital underwear is awful. My hospital have me this numbing spray called dermoplast. It was a lifesaver. Your downtown will be sore and swollen and this stuff helps tons. I would buy some in case they don't provide it. A pillow. You're toiletries if you don't want to use the crappy hospital stuff.  That's all I really needed. I'm sure more people will be along with more suggestions!
