If I were you I would just put your name on the birth certificate and keep the father unknown for now. Your better off safe than sorry, he can always add his name to the birth certificate later. However you could give the baby either your last name or his last name when born. You'd be better off doing it that way. Thats what I did with my daughter because there was an age difference. Plus we had a lawyer tell us to take the safer route.
In Texas 17 is the age of consent but if your partner is 18 then there can only be a 3 year age difference. Your dr should know the laws, or if there is a planned parenthood or something similar near by give them a call.

I would be hesitant to give someone that much power over my child at such a young age. Just something to think about. If you guys stay together and get married in a few years, you can always pay for a paternity test and change the name.
I'm sure it is more than fine for you to put his name. In my state and there needs to be a 10 year age difference or 14 years old our younger in order for charges to be pressed.
In my state of nc it has to be someone 15 or under seeing someone who is 20 or older for the state to press charges. There has to be a 5 year age difference and 16 is the age of consent. I would check up on your state laws. I think it would be fine for him to sign the birth certificate, only if you're sure you want him to have custody rights. It could be a problem later.
Every state is different but because you got pregnant before he was 18 you should be fine.
You can always change the last name. In your situation I would go with your last name.. But it the end it's all up to you.
