What could it be?

For more than a year I have had this persistant anal irritation, which I originally thought was hemmheriods (and so did my doctor). My anus and surrounding area was red and tore (tiny cuts) easily. It has been more than a year and steriod suppositories didn't help, nor did OTC hydrocortisone creams. During my physical last May (2011) I askes for an STI check to make sure it wasn't from an anal/digital encounter I had in summer of 2010. My HIV, Syphillis, Clamydia, and Gonherra test all were neg. Other than that slip up, I have been in a monogamous relationship. My doctor even did a swab of the irritation and cuts for Gonherra and Clamydia and they were neg too. I recently started using an anti-fungal creame but it seems to have made the irritation worse and now there are small red bumps flakey skin like some type of reaction. The whole irritation is around my anus and my butt crack. I asked my doctor if they thought it was herpes, but he said it didn't look herpetic (and this is from a doctor who sees a lot of STIs on a college campus).   I assume the anal swabs would be very reliable/conclusive?

Any ideas?

no std would cause persistant irritation for a year.

Thanks. Do you have a medical background? I'm assuming that this would be non-STD related since the test were all neg and the swabs too?

a dermatologist at this point would be best course of action to try to get to the bottom of this.
