Possible NGU after 3some?

Hey,
On  mid April and Late July 2022 I had a 3 some with 2 different husband & wife who are active swingers. It was all me and his wife & he was mainly just watching. There was no homosexual acts. She gave me unprotected oral & I did the same to her. There was no anal & we used condoms for vaginal sex. I have no since realized that i need therapy as i might have some type of sex addiction and I am married and worried for my wife.

A couple weeks later my wife and I had oral & vaginal unprotected sex. About a week later she had some symptoms of irritation, smelly discharge, burning pee & irregular bleeding as she is going through metopause that lasted for at least a month. She took UTI meds Anzo and then prescription (Cephalexin or Ceftriaxone). that didn't really help. She still has mild irritation while peeing but not as bad as it was.

I went & got tested multiple times and well after the incubation range as follows:

Late Sept- urine dipstick all normal
Early Oct -Chla/Gon urine, Oral swab Chla/Gono, Hep A,B and C, HIV, Syphilis and Trich - All negative

Then around 2nd week in Oct I start to have some symptoms which is about 2.5 months from my possible exposure. These symptoms last over a month and then slowly start to get better but not fully go away.
Symptoms:
1. Peeing alot like almost every hr.
2. Irritation (Stinging, sharp pains, burning and itching) inside my urethra and tip of penis.
3. After peeing I have alot of urine dripping out to the point where I have to stand and shake for almost a minute or so to get it out and still my underwear feels damn after i pull my pants up.
4. Sores in my mouth...maybe cold sores or canker not sure. Fordyce Spots all over lips and in mouth and my lips are always on fire.
4. No noticeable discharge or pain when peeing

So late Oct I test again for all the below:

Chla/Gon urine, Oral swab Chla/Gono, Hep A,B and C, HIV, Syphilis and Trich - All negative
Even test for mycoplasma Genitalium which was negative as well

Then early Dec I have my annual physical where i mentioned wanting to be test for STDS but dont say why. At this point I don't really have any noticeable symptoms.

Chla/Gon urine, Oral swab Chla/Gono, Hep A,B and C, HIV, Syphilis  - All negative
Trich test with urine- negative
HSV 1- positive which I already knew
HSV 2 negative

Urine Dipstick- normal aside from non hemolyzed 10 trace of RBC. She said that isnt a sign of infection it would be red blood cells
Urine culture- no growth
CBC- Had low WBC and low neph and  a little High Cholesterol (101)
Had me retake CBC and all came back normal aside from a little High Cholesterol (101.8)

She didn't seem concerned but referred me to Urologist if I wanted.

Late Dec- Have unprotected vaginal sex with wife and a week later similar symptoms come back along with some infrequent, minimal clear discharge. Notice when masturbating alot of watery pre-cum?

Mid Jan go see urologist 1:

I feel like i see swelling on side of testical where leg meets testical. Urine dipstick and physcial exam he says is normal. No noticeable discharge at time of visit even when milking or swelling. I bring up NGU and Urethritis and he laughs saying those aren't common.

Says follow up in 2 months.

Get 2nd opinion Late Jan from different urologist 2:

Mid Feb back to urologist (2) as symptoms persist except discharge is rare now and when it is its clear. Feels like my groin area is swollen and achy along with testicles. Still have burning urethra but not at time of visit and notice alot of bubbles in urine

He says physical exam is fine. Urine dipstick is fine and even looks under microscope and says doesnt see anything. None of these symptoms are STI related and he knows my exposure. I ask about NGU and Urethritis and he says I have been on the internet too much and those are rare without positive STD testing.

He does a urethral swab (hursts like hell during and after) for Chly/Gono which comes back negative

I have my follow up with urologist 1 and he had me take a urine test before visit. It was a post of trace of Ketone. Everything else was normal as for as WBC,RBC and everything else. Waiting on my appointment coming up now.

Sorry so long but been going on for a while. Along with my wife and my symptoms I am worried. Her symptoms came back after our sex in dec again with all the same as before. She says she feels achiness in her ovaries and uterus. She also feels very fatigued and run down.

1. What else could this be?
2. Is NGU or Urethritis still a possibility even with urine dipstick, urine culture and urinalysis coming back negative for WBC?
3. Do I need symptoms at that exact time of these tests for NGU/NSU or Urethritis.
4. Could have still have NGU or Urethritis with no testing confirming it or lets say I get a positive test of WBC but all STD tests are negative. Is that still an unidentified STD that can be passed?  
5. Could all this just be coincidence? Timming of everything just seems weird and that we both have symptoms but I cant test positive for anything?
6. Could I have caught someone unknown bacteria from giving oral to couple wife i was with and then gave it to my wife by giving her oral and then she gave it back to me when we had sex?
7. Did i get something from oral from one of wives i was with? What could it be?

Okay, so no matter who or what you caught, if you don't have elevated WBCs in your urine, you don't have NGU or urethritis.

I do wonder if your doctor has done any prostate exams.

Your wife's symptoms could be related to menopause. When women go through that, the loss of estrogen can cause thinning of the tissues in the reproductive tract, and that can cause irritation, burning, UTIs, etc.

She should talk to her doctor about possibly using a vaginal estrogen.

I think it's coincidence. You have tested negative - multiple times - for everything, and your wife's symptoms could be easily attributed to menopause.

If you had a prostate exam, and that's fine, then I wonder if maybe this isn't anxiety and guilt.

How's your water intake? Caffeine and alcohol? Both caffeine and alcohol are really irritating to the bladder, and then the urethra.

Are you in treatment for your sex addiction? If you're serious that you have that, get help for it. You've invested a lot of time and money into figuring out what's wrong with you physically. Your mental health should get just as much attention.

Thanks for the quick response!

1. As for the elevated WBC I haven't gotten one dipstick or urinalysis with that. The only one I had was the RBC non hemolyzed 10 trace. And the 2 urine test after that was nothing. The ketone trace I have gotten in my past 2 tests though. No relation to std?

2. What is the preferred or more accurate method to test for NGU or urethritis? Urethral swab or urinalysis? Does it matter? I have heard NSU and or Urethritis can happen and no pathogen be found?

3. He has not done prostate exam. I will ask next time I go.

4. I have only tested once for MGEN and that was negative. What about Ureaplasma urealyticum or Mycoplasma honis? Not sure if that is possible or common with unprotected oral and protected vaginal.

5. Dont drink alcohol that much and only have 1 coffee all day. Drink water throughout so don't think that is any issue.

6. It all just seems too coincidentally you know. The timing of it all, us both having symptoms and them coming back for both of us after sex. Could I have given her PID, BV or cervitis even though nothing can be found in me?

7. I am in the process of getting into therapy for sex addiction.
Sorry I should also mention that since these symptoms she had experience very painful sex we do have it. Not sure if that changes your mind into what she could possibly have?

You can bring me all the "what ifs" but you have continually tested negative for STIs over a long period of time. You need therapy, not more testing, unless it's prostate exams/testing. That's it. If your wife is in menopause, you must be nearing or at the age to start getting prostate exams, right? Do that, and then find a therapist.
