STD testing and treatment HELP!

Hi, I'm 17 years old and I live in Oregon. I was wondering if there was a place where I can get free STD testing and if necessary, treatment, without letting my parents know. Also, if there's a price, how much?
More specifically, chlamydia. About a year ago, I tested positive and got it treated. My mom knew about that one. But, a couple weeks later I had sex and since then, I have not been checked. I'm scared that I might have it again. And since it has been almost a year now, I'm terrified that if I did have it, it has done some damage to cause me to become infertile.
Could I possibly be infertile if I had it for this long? Where can I get tested and treated, for free or a low cost, without my parents knowing? Please help. Thanks!

If you do not use condoms you need to.

The state health dept does testing, you would have to look and see where your local office is and see when they have testing.

Lets not jump to extremes with infertility until it comes to that time. Most people will not be infertile even if they have chlamydia for a long time.
