Worried about STD
I had an encounter in Kenya, Africa, where i visited a chinese massage parlour. The lady rimmed me and then performed oral sex and then i had protected intercourse. After 3 weeks, i had burning sensation in urine and pain during ejaculation but had no discharge, no blood, no foul smell in urine. After a few days, my testicles began to hurt. I visited a urologist.He did physical examination and diagonised me with Epididmytis and also advised a routine urine test. My urine test showed no abnormalities. (My age is 43). He put me on to Paracetamol, (3*3 days) Urispas (3 * 5days) and Levofloxacin 500 mg (1- 14 days). I specifically asked him should i abstain from sex and alcohol during treatment and Doctor suggested I should go ahead my life as normal. I also asked him, if he feared an STI. He was categorical that he does not see an STI threat. After 3-4 days of medication, my burning during urination and pain during ejaculation went away, but still have a very mild pain in testicles. After 2 weeks of medication, very mild pain in testicle persists and have a very mild burning in tip of urethra occasionally. I revisited him and he examined me and said it could probably due to vericose veins and extended 250 mg of levo for 2 weeks (1 daily). Again i asked him my doubts about an STI and he confidently said no he does not see an STI. ( I have not told him about my Kenya encounter).
My question is : 1. can i rule out a STI completely or do i need to take some precautions and more tests?
2. Does the inflammation of testies take a long time to go?
3. How do i know i am fully cured and there are no side effects in future?
4. Is it reasonably safe to have unprotected oral and protected intercourse with a sex worker?

1) Yes, you can rule out a sti. Doctor had examined you twice and no appearance of sti has been detected.
2) It does take a moderate amount of time but the medication you were prescribed would have taken care of that.
3) With the decreased stress of symptoms occurring, it may indicate that the symptoms are waning however, you would need to revisit your physician for further medical evaluation and clearance.
4) You need protection even for oral with someone who is a CSW and someone you had met anonymously. Unprotected oral is low risk but not a no risk situation for std transmission. You can wind up with gonorrhea, herpes, and syphilis if one were to have them..
