Totally Confused!!!!!!

2 years ago I contracted herpes on my pelvis just above my penis while wearing a condom. My first outbreak was very small and contained to an area smaller then a dime. 2 months later while going through radiation treatment a second outbreak occurred in the same spot and same size. It has now been 2 years without a breakout until a month ago. The spot was on my anus and it didn;t feel or look anything like the first two, so at the time I thought it was something else. My girlfriend, who knows about everything, has just contracted it as a result. She has never touched that area on my body, but it showed up in the same spot for her as well as on the front side. I was under the impression that it always came back in the same spot and had the same symptoms. I was also under the impression that it was contracted by skin contact. Was I misinformed or did she potentially contract it a different way? In addition, can I now get it worse and actually on my penis if we have sex during her outbreak?
