You know, condoms are pretty effective. But I'd hate for you to be in a situation where it falls off, breaks, anything like that. Make sure you always use a condom to protect yourself from STD's as well. But if this is your girlfriend you're talking about, talk to her about using another method of birth control.

Tiny. As long as the condom doesn't come off, or split, they are pretty damned effective. Most also include a spermicide, as back-up even if a little sperm did get out somehow.
From what I remember reading, condoms have a success rate of over 99% - that means a couple, having regular sex for a year, using just a condom as contraception, will have less than 1% chance of getting pregnant. Even when it fails, it's generally user error rather than anything wrong with the condom.
