I think you should get some blood tests done, most importantly Free and Total Testosterone. If you are low in the range, your ED problems or lack of desire is natural.

Its wise to have Prolactin, FSH, LH tested as well in addition to Iron labs (Ferritin), Vitamin D/E, and Mineral Scan (Zinc, Magnesium).

OK Over masterbating is not a good thing, and I'll try and cover this first, you have not said but did you do any of this masterbating to porn, as you could have (porn enduced ED), not good, you can google that and it will tell you that you need to quite masterbating for 8 weeks, they say that it should reboot your system, and cut out the porn for good.
The smoking quit now, full stop, but cut back slowly to let your body to get acustemed to it, over 12 weeks should do it, it did for me.
Get back to exercising its so good for even if its just brisk walking and swimming.
OK now for your premiture ejectulaion, there is an exercise called edging, just go to pegym, and on there you will find out about edging.
If you still have a problem after 4 months come back to me, and I will give you a list of herbs to take, OK
Good Luck

Hi there,  well, first let me say that there are no doctors on this forum. This is a member community.  You could try an expert forum by using the tool bar above to reach a doctor.

Do you think you could be depressed?  I ask that because you are having this difficulty and also stopped exercising.  This can affect one's sexual health.  You also talk about masterbation which does seem likely to also be playing a role.  I would cut back on this experimentally to see if this helps.  What do you have to lose with that?

Have you had a physical any time recently?  Your health care provider is a good person to discuss this issue with as well.  goodluck
