My Friend has a TSH of 150 any words to help calm her

Hello,

I have been doing some of my own research on problems that might be related to me, but I do not know much about goiters/nodules/ very high tsh results.  My friend has been complaining about her neck feeling big and a lump on her neck for a while now, I didnt know this until about a week ago and I told her she needed to go get her thyroid checked.  She got it checked yesterday and of course they only did TSH with a reference of .4-4.0 her level was at 150.  I tried to calm her and tell her that it was not that out of the ordinary to have a tsh that high with a nodule or goiter.  I am assuming that is correct, however I don't know. She is already an anxiety sufferer and I am always the only one she trusts enough to calm her down.  Can anyone tell me anything based on these tests results of what type of thyroid she might be suffering from?  Is it likely hashimotos, thyroid cancer, or just a nodule ? also do these nodules or goiters secrete tsh?  Any help would be greatly appreciated and any encouraging words and ideas I can convey to her would be great.

Post partum thyroiditis, also called "silent" thyroiditis, is usually characterized by an initial hyper phase followed by a temporary (though it can last for several months or even a couple of years) hypo phase.  Did she go through a hyper phase after delivering?  Antibody tests will distinguish between Hashi's and non-autoimmune thyroiditis.  There's also "permanent" thyroiditis that is not autoimmune, but of indeterminate cause.

Any thyroiditis (inflammation) can cause swelling (goiter) or nodules (tissue overgrowths).  It's the inflammation, regardless of cause, that causes these.

Anxiety is often a symptom of hypo.  That might just explain her anxiety.

Although it's impossible to diagnose without antibodies, as you know, it's most likely Hashi's since this is the most prevalent cause of hypo in the developed world.  However, there are other types of thyroiditis that can cause similar symptoms.

The pituitary is the only thing that secretes TSH.

She needs to have FT3, FT4 and TSH tested, along with TPOab and TGab.  She also needs to be on meds.  I'm sure she'll feel much better.  Often, once on meds, goiters (swelling) reduce in size and nodules do also.  

Just reassure her that she has a very treatable disease.

I forgot to mention, she is 4 months post partum.  IT may be just due to that?  Does post partum thyroiditis often present with a lump as well and would a tsh that high be consistant with that?  She found out yesterday and is being prescribed meds. Thanks.

No Hyper Phase, but I have read it can present without the hyper phase as well.  Just wondering if tsh of 150 is alarming as far as cancer or what not.  And how common is thyroid cancer as opposed to just benign, and also how do they tell whether or not its cancerous?

Keep in mind that the TSH could flex in other odd hormonal situations. If a recent pregnancy is known the TSH is really - really a poor indicator of everything going on and it could be a stress response more than anything.


She would need to look over any recent medication she had been on to see if this could have triggered the abnormal TSH also.

Tell her not to fret. These high values can dip quickly and then all this worrying was for nothing.

After a pregnancy - the hormones can sometimes take a min of 6mths to a max of 2 years to settle back in to their normal patterns. And that is if everything goes fine and she doesn't upset the hormones with meds - foods or stress further.

In rare cases antibodies are triggers for this too but again due to the pregnancy those can also rear and fad at any time.

Cancer is NOT measured with a TSH. - A TSH can be a guideline onocologist's may use to suppress that number but everything cancer related is around a TgAb reading.

She has all hypo symptoms and her neck is enlarged with a lump... so it is thyroid related I am pretty sure.  But that is reassuring that all those other things could cause it to be that high and that it is not an indication that there is cancer. Thank you,

You might was to take a look at post pregnancy thyroiditis...

This sounds more than a classic hypo situation.

Above that's why I stated antibodies can rear up too and if they do a tender - swelling of the neck is common.

Read about that.

And the recent pregnancy can be why it flared. If you want some good advice for her - she should attempt to change a few things to see if she can re align her immune system after this shock to get things fixed some,.

I believe if she pays attention to herself on those levels it could help after that pregnancy
