Overmedicated? Please help.

I got my labs done last monday and the doc published the results on the patient portal. see below (I have an appt tommorow at 830AM for a follow up on these labs)
I dont feel like myself at all the last few weeks. I have been having the following, speciically in the last 2 weeks:
hand tremors
weight loss
irritable stomach
lack of appetite
waking middle of the night
severe nervousness and anxiety
Menstral period every 26 days and shorter duration

Labs from Monday
TSH 1.3
Free T4 1.5 (0.8 to 1.8)
Free T3 3.5 (2.3 to 4.2)

I even went to the ER  b/c of the hand tremor, anxiety, and irritable stomach and they ran all sorts of CBC, liver, pancreatic, urine, etc. Even took Xrays. All other blood tests were ideal.
Are my above results enought to bring out HYPER symptpms?

Any feedback appreciated. I'm getting scared.

Saw Endo today. She said on paper my labs and vitals are not hyper. She wants me to stay on the 50mcg synthroid. However, she reduced my Metformin and stated she felt that was the medication giving me problems with the weight loss, irritable stomach, and lack of appetite. Which was causing me anxiety (bc I didn't know what was wrong) She said if the tremor continues in my hand or gets worse, she will reduce dose. So, we will see how this week goes. I think Oldie mentioned a good point that with other meds it's really hard to pin point if your thyroid is out of wack or it's another side effect.

I agree with you.  The more things you have wrong with you and the more meds you're on and even what stage of life you're in, the harder it is to distinguish hypo from hyper.  The point I was trying to make was that we all know and espouse daily that "just being in range" often isn't enough to relieve hypo symptoms.  So, we recommend FT4 at about midrange.  If you accept that construct, you also have to accept that some people are going to feel hyper, even with in range labs. As we often say, "You're not hyper unless you feel hyper".  The other side of that coin is, "If you feel hyper, you are, regardless of what the labs numbers are."

I had symptoms similar to yours when overmedicated.  I was antsy...couldn't sit still.  I woke up a lot in the middle of the night, and I was WIDE awake, the "I can't close my eyes" kind of awake.  I had hand tremors.  The last time I was overmedicated was quite some time ago, and it was after a very small increase.  So, I caught it pretty fast.  However, by the time I was on the phone to my doctor, the triage nurse asked me to take my BP, and it was something like 180/100...very unusual for me.

Sure you can feel different but that isn't overly hyperthyroid on the labs. Your heart rate, blood pressure/pulse pressure and body temp doesn't indicate hyperthyroidism either. But you may have a mild excess of thyroid hormone causing some symptoms. You'll soon know if those symptoms improve reducing medication.  It would be interesting to see what your heart rate, blood pressure and body temp is after you take thyroid medication.

As I read your posts I couldn't help but wonder if you have been a full iron test panel:   ferritin, serum iron, TIBC and % saturation.  

Possible symptoms of low ferritin/low iron
• Achiness
• Weakness
• Fatigue
• Dizziness
• Headaches
• Faster heart rate
• Hair loss
• Foggy thinking
• Memory problems
• Loss of sex drive
• Depression
• Increased irritability
• Heartburn
• Abdominal pain
• Pica, or the compulsive eating of non-food items
• Pagophagia, or the compulsive eating of ice
• Numbness or tingling of extremities

Common Visual Signs

In addition to the list of possible symptoms, those with low levels of ferritin may also have visible signs of deficiencies that can be seen physically. These symptoms may include:

• Easy or excessive bruising
• Paleness of skin
• Brittle nails
• Sores at the corner of the mouth

That is sane and serious comment. My only reservation is for those who have other problems as well as thyroid and are liberally dosed for high blood pressure, high cholesterol, heart arrymthias, anxiety,etc because the side effects of medications for all these and many others prevent you from distinguishing symptoms of hyper. I have been trying to get to the right levels for 5 years. in 2011-12 I took from 150mcg down to zero levothyroxine cutting down 25mcg every month or six weeks before retesting. There was no discernable result. FT3 stayed below the minimum. I have the same problem with HDL (good cholesterol) it stays below the lab's range of 40 minimum. Meanwhile, all the usual symptoms continue - dizzies, belly and headache, imbalance, weakness, fatigue, nausea and so on despite exercise ,diet etc.

Goolara, can I ask what your HYPER symptoms were when you were overmedicated?

Hello goolara, I take synthroid 50mcg, since June for mild hypo.  MY Endo hasn't cut dose down yet, but believe me I have an appt at 830 tomorrow and I'm going to beg her. My hands tremble unbelievably bad especially after I take my morning dose. I'm losing weight for no reason and I feel extremely shaky and nervous especially first half of day.

Each person does feel different at different levels.  I can never even get close to the midrange FT4 and upper half of range FT3 guidelines without feeling very hyper.  I've felt hyper well below both those guidelines.  Trust your symptoms.

Your FT4 is a little on the high side.  It's 70% of range, quite a bit above the 50% recommendation.

What are you taking for meds?

My temp was around 98.0 when I went to the ER.
BP was normal too. 120/80
I think hearteate was 84.

I'm wondering if each person feels different at different levels?
