thyroid

my free T4  is at 0.5  range is o.8-1.8
my tsh is at 0.24  range is 0.40-4.50

one month later they did Not  test my free T4 but they tested my TOTAL T4, I don't know why. so my Total
T4 is 3.6,  range is 4.5-12.0
my TSH is 1.42 range is 0.40-4.50,  I  had not taken any meds  Why did it go up?  The only difference was this test was taken in the morning.
I had a thyroid ultra sound and they found one 3mm nodule, the gland size is normal and homogeneous in echotexture and the isthmus measures measure 2mm ap diameter.
What does all of this mean.
I see an endo tomorrow any questions I should ask?

Did he say anything else about what he thinks is going on?  Was there an explanation for your very low FT4 and high FT3?

With your FT4 so low, it's only a matter of time before your FT3 starts to plummet.  

Tough question...I can understand that he might be afraid to put you on meds now with your high FT3.  Meds could put your FT3 over the edge.  On the other hand, your FT4 is extremely low.  I'd have a tendency to think two months is a bit long to wait.  In a month, you could see if either or both FT3 and FT4 are trending.  Also, it would give you another FT3 reading to add to your history.  I'd certainly be on the phone in a heartbeat if your symptoms change between now and your retest.

What do you think?  How bad do you feel?  Can you take two more months of this?

No he's not suggesting meds. but will retest in Dec.

So, your FT4 is below range, clearly indicating that your thyroid is not working well.  However, your FT3 is at 89% of range, very high.  This would explain why your TSH is low.  When FT4 gets very low, the body tends to "over" convert T4 to T3 in an effort to maintain useful thyroid hormone levels.

TGab and TPOab (microsomal) are both negative, so it appears you don't have an autoimmune disease.

Is your doctor suggesting meds?

Yes, I do need the ranges.  Ranges vary lab to lab, so you have to post both together.

I went to my endo yesterday and he's having  my free T3, freeT4/free thyrox, TSH, thyroglobulin AB, microsomal AB, and thyroxine binding globulin tested.  I should know the results tomorrow. I'll let you know what I find out.

Yes, when both FT4 and TSH are low, it often indicates pituitary involvement.  When both are low, it can be secondary hypothyroidism, which is actually a pituitary dysfunction.  The pituitary doesn't put out enough TSH to stimulate the thyroid to produce hormone.

With both FT4 and TT4 below range, one would expect your TSH to be much higher.  FT3 wasn't tested, however, and if your FT3 level is high enough, that could explain the discrepancy between FT4 and TSH as well.

I do have symptoms....fatigue, weakness, weight gain,
I was wondering if both free t4 and tsh were low does it have something to do with the pituitary gland?

Both your FT4 and TT4 are below range, indicating that you might be hypo.

TSH can vary as much as 70% during the day, just depending on the time of day the blood was drawn.  In the first set of labs, was your TSH 0.24 or 2.4?

How do you feel?  Do you have hypo symptoms?
