Hashimoto's with multiple swollen nodes

Background
27 female

Diagnosed with hashi's 4 months ago, swollen left lobe, 3 nodules (2mm, 2mm, and a 5mm), endocrinologist said antibodies were "out of this world high", all other numbers were normal.  Started synthyrioid 100, fine for 2 months, developed chest pain and racing pulse, had to cut dose to 50.  Starting to climb back up.

I am having swollen lymph nodes in my neck, under arms, and around right breast.  Some stay for only hours, others stay swollen for days, all are painful.  I told me doc about this, he said neck was due to Hashi's, but wrote off the others as having some small infection that triggered swelling.  That was 4 weeks ago, swelling is still happening.  Since the last bad flare up, I am also having a weighted feeling on my chest radiating from my sternum.  Sometimes the pressure combined with the choking feeling of the swollen thyroid makes it hard to breathe.

Question
Should I be concerned about this swelling, does it lend itself to cancer, be it thyroid, lymphoma, or breast cancer?

Back at the docs in 2 weeks to try and boost meds again, should I be asking for an MRI or something??

At reg doc app my tsh was a little high but not bad, I was on synthryiod 100, 1 week later I started to have heart issues, so the doc cut back to 50 without checking the numbes again.
