Help in diagnosing symptoms

I have a coworker whose sister, 38 years old, is extremely ill and doctors cannot diagnose what is wrong with her.  This poor woman has been having a high fever and cannot hold anything down.  Her symtoms are high fever, vomiting, muscle and bone pain.  Doctors have checked everything and are baffled.  She is in the hospital and has lost a lot of weight as you can imagine since this has been going on for three years.  She will all of a sudden get a bit better, then goes back to being very ill.

Any information would be helpful.  Thank you.
