This is just a suggestion--have you ever had an MRI of your neck and/or jaw?  You could have some problems in your cervical spine.  I have herniated discs in my neck, which cause jaw pain, shoulder and upper back pain.

As far as the teeth, I have had a lot of root canals and extractions and sometimes the tooth bothers me for years before it goes away.  You could try a periodontist and see if they see anything in the gum area.
