Burning tongue can be caused by a b12 deficiency. You should be tested for b12 levels and if they are even border-line you should take b12 supplements.

The burning tongue can be due to menopause like condition caused by PCOD or hormonal disturbances, or due to vitamin B12 deficiency. You may not have any other symptom of Vit B12 deficiency if it is in its initial stage. Iron deficiency and Vit B6 or pyridoxine deficiency may also be associated as the tongue swells on a bad day. The other possibility is allergy to some food preservative or to propylene glycol which is found in some food items, drugs and cosmetics. Maybe your dental paste has it. Check this out. Possibility of any drugs causing it or dry mouth, oral cancer, damage to the lingual nerve, and diabetes are the other causes for a burning tongue and if nothing else is found these should be explored. It can also be gastric reflux (ruled out in your case. If all common causes of a feeling of something stuck in the throat or a foreign body sensation in the throat has been ruled out (post nasal drip, gastric reflux, allergy by a skin test or RAST panel, food allergy), then rare causes will have to be looked into. One is upper esophageal dysmotility—can be known by endoscopy and barium meal. Projecting superior cornu of the thyroid cartilage is another possibility and surgically trimming it can help. Esophageal candidiasis or fungal infection, gonococcal pharyngits, chemical burn of esophagus due to acid, and cancer of the esophagus are some other causes. Another possibility is irritation of the anterior fibers of C2-3 spinal nerves with spasm of the muscles attached to hyoid bone. This usually happens due to injury or postural problems causing curving of lower cervical spine. If everything is ruled out then the problem could be psychogenic and counseling may help.
