Hello,
In some circumstances, imperforate anus can effect the kidneys via the urinary bladder and ureters and may cause renal failure. This all depends on the presence of fistulas between the rectum and bladder.
During the growth of the baby in the womb, the rectum and anus develop from the cloacal cavity. There is a partition called urorectal septum in the midline. This septum separates the rectum and anal canal from the bladder and urethra which lie in front. The cloacal duct is a small communication between the rectum and bladder. This urorectal septum grows down to close this duct by 7 weeks' gestation or pregnancy
Non closure of the cloacal duct causes rectourethral fistulas or rectovestibular fistulas. This can cause fecal matter to communicate with the urethra or bladder in case of imperforate anus.Hence it causes infections of the bladder and thus kidneys.Repeated infections can cause renal or kidney failure.
Hope I have provided you with sufficient information. In case of additional queries, pls post us.
Take care and God bless.
