someone please help me

Hello,

I am 24 years old but I feel like I am 74.  I was diagnosed with a hiatal hernia a few years ago because I couldn't stop vomitting for almost a week straight.  After 2 endoscopies, I was told I had a hiatal hernia.  I have been taking Zegerid 40mg on and off for a few years.  There are days where I will not have any symptoms and other days where I doubt it is my hernia because it gets really bad.  These past few days have been horrible.  I take the Zegerid and I am still suffering from pains I am unfamiliar with.  I feel as though my breathing has gotten difficult and that I cannot get in enough air.  The burning is in my upper abdominal area just below my breastbone. Could this be something else?  Possibly an ulcer? It is scaring me that my insides are burning and it does not allow me to sleep well at night.  

Nonetheless, In June I awoke one morning with severe joint and muscle pain accompanied by swelling.  After blood tests in the ER they said my only abnormal blood result was postive 2 for ra factor. I was put on prednisone for 5 days and I saw a rheumatologist a few days later and was told it could be a virus and 2 is too low for it to be rheumatoid arthritis and it has to last more than 6 weeks for him to even consider that.  Two days after seeing the rheumatologist I went into anaphalactic shock and was put in cardiac telemetry overnight.  I was still having joint paint.  The rheumatologist came to see me in the hospital and said that it could still be a virus and that all my blood work looked fine.  He put me on 2 weeks of methylprednisolone steroids and that literally turned me into a vegetable.  I was placed on a month or so of disability because I was literally disabled. In the mean time, I went and did a full on allergy test on my whole back and found I was allergy to absolutely nothing so that was still undiagnosed as to why I went into anaphalactic shock.   I had to go back for testing in a month and the new test results revealed that I am low in vit d about 13 was my blood result so he put me on 50000 units each week for four months.  I just took my last vitamin d pill two days ago and I still have joint and muscle pain and /evening swelling.

I have the worst quality of life because now this burning in my throat, esophagus, and upper abdomen won't go away accompanied by breathing issues now and my joint issues.  What is wrong with me? No one can diagnose me. Is it a combination of multiple things?  Could I have gallbladder issues because my dad had his removed? HELP!

Since you have gut issues, joint pain and low Vit D rule out H pylori infection (by carbon urea breath test) and Crohn’s disease. Crohn’s disease is a difficult entity to diagnose. Along with gut symptoms skin disorders and arthritis too are commonly seen with Crohn’s. An upper GI series using barium dye is done to see the condition of small intestines. The degree of involvement of the large and small intestine may vary from person to person.  A biopsy is the most confirmatory. An elevated ESR, low protein, minerals and iron in blood are the other findings in Crohn’s which may or may not be present. A recent advancement in Crohn’s diagnosis is video capsule endoscopy.
Please consult your gastroenterologist. Also get your thyroid and parathyroid gland tested as you have low Vit D and serum calcium levels checked. Take care!

It's not your gall bladder...the symptoms are not compatible. If you had two endoscopies and they didn't see ulcers then you don't have them. You might ask your GI specialist if they tested you for H. Pylori ..an infection that can produce symptoms and needs a antibiotic therapy. If they didnt test you for it you can get tested  with a simple breath or blood test..You should ask rheumatologist if you had a full autoimmune panel..including for scleroderma. If zegrid doesn't help then the burning is not likely acid...but switching to another agent like nexium..or the cheaper OTC med Prilosec or protonix might work better. Antacids for the acute burning (milk of Magnesia or Maalox etc) is effective if burning due to acid or bile salt irritation.
