Thromboses vein after varicocele surgery.
Hi everybody and thanks in advance for the help.
I've undergone a varicocelectomy on my left testicle. It was done by ligation of the veins, and the incision was made just a couple of inches away from the penis.
RIght after the operation I've noticed that the dorsal vein on my penis had like few grains in it and the vein looked hard like a cord (i dont know if it is a vein or a limphatic vessel) ans slightly sore. The clots are right at the base of the penis, I can feel the 'cord' going inside my body in the direction of the scar of the varicocelectomy. The doctor told me it was a thrombosed vein and that would go away by itself with some time. A month later was still there. The doctor pescribed me an ultrasound and checked the area around the incision he made, a small inflammation of the spermatic cord was found, and some little edema under the scar. Regarding the thrombosed vein they said it was a flebitis (!?!) but it had resolved and now only the blod clot remained. They told me it would have gone away buy itself, no treatment was needed.
Now other 2 weeks have gone by, and my vein is still there, on top of it now it reaches up to the glans of my penis and another small vein that starts from the thrombosed one seems to be harder and sore too.
I've had sex meanwhile, because nobody told me not to.
The surgery was done in italy, now I am back in the states where I live, but have no insurance at the moment.
I know, that noone can solve my problem by emails. But i hope someone could give me their opinion. I am going to take aspirin from today on, as I've read it is ablood thinner.

Hi,

Phebitis is an inflammation of a vein. It can occur secodary to trauma, indwelling IV cannulae, etc.

If the inflammation is in response to infection, you will need antibiotic treatment to prevent progression of the disorder. If you have had antibiotic treatment, other surgical intervention is not warranted.

As you have had surgery nearly 2 months (?) back, it would not be advisable to start aspirin without the supervision of a physician.

It can cause risk of complications such as bleeding in the area you had surgery, as the tissues are still healing.

If you have any hypertension, you should not use any aspirin without your physician's advice in view of the risk of hemorrhagic stroke.

Aspirin is effective in thrombolysis with low dose treatment (75-150mg per day) and not at the regular dose of aspirin, that is, 350 mg.

I have been operated of vericocele in my left testecle.Today is 9th day and I m seeing a large clot of viens in the area where my penis start..i m very tense 4 all that..its painfull wht the hell is happing with me..
Pls give me some suggetion ,thanx in advance

Hi! Dr. I was recently operated a month ago of varicocelectomy on the left side and all seemed well, til yesterday when I saw my incision and observed a tiny hole in the center to the right at first it bled a bit but after placing a paper towel on it then it stopped, I placed some alcohol on it and triple antibiotic, so then it stopped bhleeding, today I still see the whole, though its not bleeding but looks pinkish and moist. Is this a sign of soemthing bad or dangerous? I dont feel pain on the area since it is all numbed up after all of the nerves in the area were cut up. Would appreciate your feedback, thanks!

i had varicocele surgery done 1 year ago.
and the veins never went away. But i have read that i shouldnt have to worry about them. im 17 years old. so why is this happening?

wud like tp consult you . had undergone a varicocelectomy . The pain increased post surgery.Have increased veins 3 months post surgery .

1. First observed 14 hrs back  in the under scrotum as a bag of worm.

2. No pain till i got the surgery done.

3. Got surgery done to get rid of swelling.

4.
(a)Pain started with more fibrous structures along the cord on left side.
(b)Pain at the penile root on either side(had a bilateral varicocelectomy, though no symptom on right i had before )
(c) burning sensation on the penile skin on the left sometimes .please suggest

      i have been operated on varicocele  on my left testicle in 2007.i want to workout on my body.i want to know whether i can left weights.if so what are the precautions should i fellow and what kind of diet should i maintain.thanking you and expecting a reply.

Hi,

Good to know you are healing well.
Keep your diet balanced for vitamins and other micronutrients and I guess about now you would be able to get back to your earlier levels of physical activity.

HI doctor,
3 months have passed since I've had surgery. Now everythings seems to  be good. I have no pain anymore. I dont feel pulling in the area around the incision anymore. I still have a bit of thrombosed vein on the tip of my penis, but I guess it will take some more time to go away.  
I've seen a doctor, I've had an ultrasound done and everything appears to be healed fine.
I'll keep you posted if anything changes.

Thanks doctor, you have been of so much help and answered to all my questions better than the doctors that I've been talking to in italy.
I am happy that people like you so passionate about their job exist.
I haven't been taking medicines so far. The vein seems to get thinner...I hope it keeps on reducing in size.
The area under the incision its still sore.
Overall I think I might be getting better.
I will keep you posted.

Hi,

No, I was talking about the vein that apparently had phlebitis - What appears, from your post, to be the dorsal vein of the penis.

Indomethacin is an antiinflammatory agent and helps in faster resolution of clots (thrombus).

Recanalization means that the thrombus would have been broken down by the cells in the blood that are specific for this function.

No it will not be necessary to abstain from sexual activity for as long as 6 months. Just so long as you don't feel sore.

Better to wait a few weeks (6-8 weeks) after the varicocelectomy ato resume normal gym activity. Also build up gradually to performing resistance exercises that may strain the abdomen.

I will look into the indomethacin and if it available without prescription...but i doubt it.
Now I am a little confused by your last post. When you say that the recanalization of the thrombised veins takes up to 6 months, you mean the veins that have been ligated for the correction of my varicocele right? Not the dorsal vein of the penis that is actually thrombized.
Also, if it will take up to 6 months to resolve, does that mean that I have to avoid sex untill then? I mean, is that even possible? Someone can't just stop having sex for 6 months...at least I hope not.
Another thing: usually after one have had surgery like mine (varicocelectomy) will it be possible for him to go back in the gym to work out? I know now its too soon but in a few months will it be possible?

Hi,

It has been advised that sexual activity should be stopped until resolution of symptoms in superficial phlebitis of the dorsal vein of the penis.

Additionally, you can start taking indomethacin (Indocin) 50 mg thrice daily, to help with the pain and to reduce inflammation.

The recanalization of the thrombosed vein can take anywhere between 3 weeks to 6 months.

So, give your organ the rest it needs.

Continue the indomethacin for a week and then reduce the dose to twice daily.

Hi doctor and thanks so much for taking the time of answering my questions.
It's now been a month and half past surgery. I did take aspirin for a few days but nothing happened so I stop taking it. I did not know that for the thrombosys a low dose its usually used...its good to know in the future. I agree with you of not taking aspirins anymore without consult a doctor.
I am just extremely worried and confused. I am trying to get insurance here but its really hard and costs alot. I have appointment in a month with a free clinic doctor...in a friking month.
The hard vein is still there, altho the part at the base of the shaft is thinner than before, but the top part its thicker and the other vein that starts from the one clotted is very enlarged. It looks like a lump, and hard lump but when i touch it i can feel the structure of a vein or vessel.
Lately I had pain after urinating. And also pain on the sides of the base of the penis, the pubic area, feels sore. The weird thing is that it hurts from both sides not only the one I had surgery on.
My other problem could be that I've found out I have an addiction to sex. Therefore I cant keep it at rest for more than a few hours.
I can't wait to be able to se a doctor here in the US. Meanwhile I do apreciate a lot your suggestions Dr Mathews, and I will keep on posting here so that in the future someone with the same problems can use this informations at his advantage.
