prostatitis

I had a colon resection 3 years ago due to diverticulitis. since then my psa went to 4.5 and with anitiotics came down under 4. now a year and a half later it went up to 5.8 with some groin pain and some burning. My intern did a DRE and said normal. He gave me 30 days cipro. I saw a difference for the first two weeks and then it went back to symptoms My urologist said he felt a soft but rubbery spot which he felt 1.5 years ago and seems the same, and also said I had micro blood in urine . he gave me cipro but I asked for levequin for 30 more days. Urologist wants to do a biopsy, a cat scan and a cystoscopy.

I hesitate to have these tests if the psa comes down , if it doesn't then I would have them.
I asked him about a psa3 and he said he doesn't rely on those.I am 55 with no other history.

Hi
Thanks for writing to the forum!
The most common cause of blood in urine is UTI. The next possible diagnosis is stone. The stone can be anywhere in the urinary system from kidney to urethra. Cancers of urinary system or some drugs too cause blood in urine. Bladder infections and cysts too can cause similar symptoms. If you are expecting your period  chances of endometriosis cannot be ruled out. Your urologist will run a couple of tests including urine examination with culture sensitivity, a CT/ultrasound of abdomen to rule out stones and other causes. These non invasive tests should be tried before cystoscopy. Also after you stop your antibiotics for 3 days get a culture sensitivity done. Many a times a simple UTI does not respond to an antibiotic that the organisms are resistant to.
I think you should discuss these possibilities with your doctor. It is difficult to comment beyond this without examining. A comprehensive investigation is required keeping all the points in mind. Please let me know if there is any thing else and do keep me posted. Take care!
