First and foremost I am SO sorry you had this happen to u. It can feel like a violation of trust! I had a dr put a speculum inside me (before I had children of course) and rip me. It was embarrising and I felt violated like u described. You definately need to report this! This is just my opinion. Just wanted u to know this was NOT your fault and you are not alone.
When she couldn't get the speculum out she pushed it further in and I felt it hit something. I am not sure if it was my cervix, but whatever she hit it hurt. so much I pulled away. She kept saying slide back down,and continued to open the speculum wider and wider while pushing and pulling and twisting. I couldn't even believe what she was doing and begged her to stop but she just kept trying to remove it without even trying to apply lubrication. I am still really upset and have a lot of pain in my lower pelvis, front and back, and it burns when I pee.

No. She just kept pulling and pushing and twisting the speculum. I could hear panic in her voice as she kept saying "I just cant get it out. Because I was shaking so much she kept telling me to breath and push. Childbirth was easier and less painful than this exam.
I'm sorry you had such a bad exam. I don't think inserting a speculum without lubrication is good medical practice. However, the speculum goes into the vagina, not the cervix. The cervix is tiny and the speculum cannot go into the cervix.
Did she not even apply lube after she couldn't get the speculum out?
